About the project
Greater Manchester's innovative service model, based on the idea that to improve health requires tackling the social causes of ill health alongside clinical interventions. New roles and productive relationships have been formed, between service provides, local authority, charities and community organisations, resulting in targeted, efficient and inclusive services.
