About the project
Informed by detailed research into local health inequalities, this Yorkshire-wide programme was designed to ensure isolated and vulnerable individuals have equal access to health care and support. It facilitates health providers, councils, charities and community organisations to collaborate on targeted delivery, with local leaders acting as co-producers to achieve better health outcomes for vulnerable groups.
